What it means
Means something is really annoying you, stressing you out, or pushing you to the edge. You use it for people, noise, tech, chores, anything that keeps pecking at your patience till your brain wants to clock off. Very common for a proper fed-up moan.

Where it comes from
When something does your head in it wrecks your head, grinding away at your patience until you could scream. The repetitive noise, the maddening puzzle, the person who simply will not stop, all of it gets right inside your skull. Pure British and Irish exasperation.
