Means knocking someone clean out with one heavy shot or a brutal hit, usually in a fight or combat sports chat. It plays on the heart monitor flatline image, so it's meant to sound dramatic on purpose. You use it when somebody doesn't just lose, they get switched off in one nasty moment.
