What it means
Unhinged is wild, chaotic and totally without a filter, behaviour so out of control it is almost impressive. Often said with delight about someone going gloriously off the rails.

Where it comes from
It comes from the literal idea of something coming off its hinges, especially a door that's no longer fixed and starts swinging loose. In English it first lived in that physical sense, then shifted into describing a person, rant, post, or vibe that feels mentally or emotionally off the rails.
