What it means
Means you absolutely smashed it and did way better than anyone could moan about. It comes from baseball, where a batter belts the ball clean out of the park for a home run. These days people use it for anything that lands big, from a presentation to a date to a job interview.

Where it comes from
It comes straight out of American baseball. If a batter sends the ball out of the park, that's a home run, the big shiny hit everybody notices. By the mid 1900s, people in US English were already using it figuratively for anything done brilliantly, and it stuck way beyond sport.
